Insurance Plans
Scott Norris, LMT is a credentialed provider under OSU insurance and the Ohio Bureau of Worker’s Compensation. He can accept the insurance plans listed below.
OSU Health Plan
The OSU Health Plan includes medical massage as a benefit for covered conditions, up to an annual dollar limit. Prime Care Advantage and Prime Advantage Plus subscribers carry a $20 co-pay. The Prime Advantage Value option is a cost sharing plan, subject to co-insurance (based on a percentage) and an annual deductible.

To receive medical massage under your insurance plan, you will need to provide a prescription from your doctor for one or more covered diagnoses. For ongoing treatment, the prescription needs to be renewed each year.
OSU Student Health Insurance
The OSU Student Health Insurance plan includes medical massage as a benefit for covered conditions, up to an annual dollar limit. It is a cost sharing plan, subject to co-insurance (based on a percentage) and an annual deductible.
To receive medical massage under your plan, you will need to provide a prescription from your doctor for one or more covered diagnoses. For ongoing treatment, the prescription needs to be renewed each year.
Worker’s Compensation (BWC) Insurance
Medical treatment for injuries resulting from a person’s job is generally covered by Worker’s compensation insurance. If your condition is a Worker’s Compensation case, pre-authorization is needed. You must file a claim through the Ohio Bureau of Worker’s Compensation (BWC) and obtain a Request for Medical Service Reimbursement (C-9) form from a licensed physician/provider, prescribing massage therapy. Med Pay (Auto)
Automobile insurance medical coverage (Med Pay) pays 100% of any necessary medical care required for injuries that are sustained in an automobile accident (up to the policy limit), regardless of who is at fault for the accident. You can check your policy to determine if you have Med Pay coverage and find out your policy limit, or we will be happy to check for you.
