Massage Order Form

This is a form your doctor can fill out for you to obtain exemption from sales tax for your massage therapy sessions. The doctor may also write a massage order on his own letterhead. Please do NOT use this form if you plan on using your insurance. In that case, you will need to provide a regular prescription for one or more covered diagnoses.
